多數思覺失調症患者都曾經歷一段高風險狀態,許多研究指出精神高風險個案早期治療之好處,但因個案除受前驅症狀及較差的生活品質影響,還會因成本、可用性、便利性及社會支持等問題,進而無法有健康生活的能力,導致轉變成精神心理疾病高達36%。受到新型冠状病毒肺炎 (COVID-19)的影響,各國醫療人員開始發展線上諮詢平台,且多數研究顯示其介入措施是有一定的療效,但卻極少了解精神高風險的個案在線上平台中的經驗。本研究目的為了解精神高風險個案使用線上健康促進方案平台的經驗、對話型態及對談內容意涵。採描述性質性研究法 (Descriptive Qualitative Design),依據現有已完成研究的34

位精神高風險個案,將其線上健康促進方案對談文本做資料分析。在人口學屬性上,男性13位 (38.2%)、女性21位 (61.8%),平均年齡為25.4歲 (±7.15)。其中有吸菸者為6位 (17.6%)、飲酒者為5位 (14.7%);個案曾因壓力或其他心理困擾而到醫院接受協助或治療者高達15位 (44.1%)。研究結果採內容分析法,彙整出六大主軸與二十一個次主軸,包含:一、貼圖提供始與終的訊息:打招呼/接受之立即性貼圖、表達正向感受之幸福貼圖、思考與請求之可愛貼圖、負面情緒凸顯之貼圖;二、線上健康諮詢:睡眠障礙、免疫失調、詢問醫療諮詢、可識別的精神狀況;三、情緒抒發的安全管道:負向生活事件、輕

度負向情緒、中重度負向情緒;四、有效率答詢疑問:回覆APP使用狀況、答詢方案內容、訊息互動內容;五、體認自身生活型態:飲食狀況、菸酒習慣、執行運動狀況、調整身心;及六、發揮身心潛能實現自我目標:興趣及目標、正向言談、情緒穩定。整體而言,精神高風險個案多數缺乏健康生活型態與社會支持,透過長時間使用線上健康促進方案,逐漸能夠表達正向言談、或身心健康。因此透過此經驗、對話型態、及對談內容意涵之分析結果,未來若建立符合其特性與需求的人工智能應用程式,將對精神高風險個案及醫療人員達到一定的益處。關鍵字:線上諮詢、健康促進、精神高風險
