背景與目的：認知功能障礙為失智症最主要的問題，有效評估患者的認知功能障礙，將有助於臨床診療。簡易智能量表 (Mini-mental state examination, MMSE)、簡易智能評估量表 (Short Portable Mental State Questionnaire, SPMSQ)、蒙特利爾智能測驗 (Montreal Cognitive Assessment, MoCA)和聖路易大學心智狀態測驗 (Saint Louis University Mental Status Exam, SLUMS)為四種常見針對失智症患者認知功能障礙篩檢的評估工具，且先前研究雖然有個別驗證四

種篩檢工具之信效度，但無在同一個失智症樣本中進行分析這四種評估工具之再測信度 (test-retest reliability)和收斂效度 (convergent validity)，也未有計算最小可偵測之變化值 (minimal detectable change, MDC)。因此本研究目的為同時檢驗並比較MMSE、SPMSQ、MoCA和SLUMS四種評估工具用於失智症患者之再測信度、收斂效度和計算MDC值。研究方法：本研究募集60位精神病況穩定的失智症患者，每位患者都提供MMSE、SPMSQ、MoCA和SLUMS四種評估工具進行施測，施測包含前測與後測兩次測驗，前測與後測之間將間隔兩周。本

根據衛福部統計醫學中心加護病房放置氣管內管病人約佔52.19%，因氣管內管放置造成病人無法以言語溝通，必須透過非語言肢體語言、寫字板或溝通圖卡等方式進行溝通表達個人需求或與他人互動，但受限於病人疾病因素如手受傷、肢體癱瘓、不識字等因素，往往讓病人無法充分表達而形成無效溝通，甚至可能造成醫護人員對訊息解讀錯誤，影響照護品質。因此提供氣管內管放置溝通需求開發適用之溝通輔具，是解決病人溝通困境的重要策略之一。本研究主要目的是開發高科技溝通輔助工具並應用於加護病房氣管內管留置病人改善溝通之成效探討。研究方法採實驗性研究設計，以隨機分派分配至實驗組和控制組，實驗組介入溝通輔助App.，控制組為一般常規

溝通方式。研究對象為北部醫學中心教學醫院加護病房氣管內管留置病人進行收案，符合條件且簽署同意書共52人，實驗組與控制組分別為26人。研究工具包含溝通困難度量表、醫院焦慮憂鬱量表及溝通滿意度量表，兩組採前後測方式進行資料收集，以SPSS for windows 22版套裝軟體進行資料處裡與統計分析。研究結果發現實驗組與控制組在溝通困難度皆顯著改善，再以ANCOVA進一步分析，其實驗組使用溝通輔助App.明顯優於控制組。在焦慮、憂鬱及生命徵象部分，兩組組內皆有顯著改善，但兩組組間則無顯著差異。在溝通滿意度部分，兩組組內皆在滿意至非常滿意之間，但組間並無顯著差異。本研究發現研究結果顯示溝通輔助App

.工具對於氣管內管放置病人是有助於改善溝通困難、焦慮憂鬱情形，能穩定生命徵象且病人對溝通表現感到滿意。從研究結果得知溝通輔助工具是醫護人員與病人溝通管道的另一項代替工具選擇，期望藉此輔助工具能讓病人有其他的選擇溝通輔具，有助於病人順利表達需求，以提升照護品質。周時護理人員對於因治療因素無法溝通之病人於早期評估溝通能力與溝通方式，在適當時機提供溝通輔助工具，讓病人有機會為自己的治療決策發聲表達想法，增加病人自主性。
