研究背景:棒球運動在全世界擁有相當悠久的歷史,在一場球賽中,投手扮演至關重要的角色,為了壓制住敵隊的打者,必須要投出更快的球速或刁鑽的球路,都可能會讓上肢關節需要承受更大的負荷,導致投手的受傷率一直居高不下。過去研究顯示不適當的投球機制、核心功能和柔軟度不足,以及由於專項運動而導致的投擲臂過度使用,被認為是造成球員疼痛且增加手肘尺側副韌帶重建的原因。而先前學者大多針對介入核心訓練後的球速以及核心肌力的改變,但較少文獻將投球機制納入研究要探討的目的之中。研究目的:本研究的目的是探討六週核心訓練對於高中棒球投手之胸椎旋轉活動度、核心肌力、運動學及動力學投球機制的影響。研究方法:本研究共收錄10名
高中棒球投手,依照訓練前投手於前導腳著地瞬間,骨盆的旋轉角度和軀幹骨盆的分離參數(Trunk-pelvic separation)進行分組,若骨盆旋轉角度大於-70度且軀幹骨盆的分離參數小於-25度,納入非過早軀幹旋轉機制四人(N′ETR);若骨盆旋轉角度大於-70度且軀幹骨盆的分離參數大於-25度,納入過早軀幹旋轉機制六人(Early Trunk Rotation;ETR)。於實驗開始之前,受試者及受試者家屬皆簽屬高雄醫學人體試驗審查委員會審核通過的受試者同意書,之後進行胸椎旋轉活動度以及軀幹核心肌力的測量,並架設八台攝影機來收取動作分析實驗資料。在投手身上標記41顆反光球,做完團隊規定的熱
身後,投擲10顆直球以進行動力學和運動學的後續分析。再來進行六週的核心訓練,每週三次,主要訓練時間為35分鐘,每次訓練共8個動作,並於訓練完後重複訓練前所做之測驗。統計分析使用無母數統計的Wilcoxon Signed Rank Test分析三組訓練前後的受試者基本資料、胸椎旋轉活動度、核心肌力、運動學和動力學的差異。研究結果:在胸椎旋轉活動度的部分,Post-all的坐姿胸椎旋轉活動度雙側有下降的趨勢。而軀幹屈曲和伸展的肌力皆有增強的趨勢。投球機制於各個時期的表現:在前導腳著地瞬間,訓練後的肩關節水平外展角度、軀幹側傾角度和膝關節屈曲角度有顯著的下降;在手臂準備時期,Post-all和Pos
t-N′ETR的最大肩關節內收角速度、肩關節近端力、肩關節垂直內收力矩以及肘關節內翻力矩都有顯著的下降;在最大肩關節外旋瞬間,訓練後的軀幹前傾角度和膝關節屈曲角度有顯著的降低;在手臂加速時期,訓練後的最大肩關節水平內收角速度、肩關節垂直外展力矩和肘關節屈曲力矩也有顯著的下降;在手臂減速時期,肘關節屈曲力矩都有顯著的降低。結論:本篇研究發現核心訓練確實可以有效改善投球機制,降低軀幹過早旋轉的情形,減少關節負荷,可能降低受傷風險。
